Clinical Social Worker - 224737

Medix is looking to hire a Behavioral Health Social Worker (ACSW) to join a well-established organization in the San Francisco, Antioch, and Stockton, CA areas.
Oversee a caseload of patients receiving dialysis treatments, seeing 6-8 patients per day Establish strong rapport with each patient and meet with them on a quarterly basis Provide daily treatments/sessions with clients in a clinic setting Work with a multidisciplinary team of doctors, nurses, dieticians and more to ensure the ultimate care for the patient Provide detailed documentation of each patient meeting in a timely manner, following organization's outlined protocols Patient meetings will include check-ins for concerns and current needs, intake assessments, surveys, and overall case management Assist patients with logistics to ensure they are able to attend their appointments such as transportation or assistance with scheduling follow up appointments Educate patients on dialysis and adjusting to the process, demonstrating compassion and patience Interact with family members and help answer questions and concerns around the treatment Requirements / Experience BBS registered ACSW, Master's degree is Social Work/ Psychology 1-2 years of experience in the Social Work field Preferred Experience with dialysis patients Technology/EMR Proficiency Monday- Friday 8:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
