COVERAGE REQUIREMENTS Family Medicine (FM/IM) Start on February 19th - End date around August 17th Shift & Call Details:
M-F, 8am-5pm Credentialing:
2-3 weeks POSITION DETAILS Family Medicine (FM/IM) Settings:
All - Inclusive Care for the Elderly (PACE) Patient Volume:
4-6 Required Procedures:
provide primary care and acute illness evaluation and treatment for participants in PACE program environmentCoordinate with the Interdisciplinary Team (IDT) to develop a comprehensive care plan for each participant. The IDT is composed of the following members:
o Primary Care Physician; o Registered Nurse; o Master's Level Social Worker; o Physical Therapist; o Occupational Therapist; o Recreational Therapist or Activity Coordinator; o Dietitian; o PACE Center Manager; o Home Care Coordinator; o Personal Care Attendant or his or her representative; o Driver or his or her representative EMR:
