Summary:
The Health Information Representative I is responsible for supporting the clinical Next Gen Document Management system primarily for the Inventory of historic paper records and basic scanning of documents into the Electronic Health Record (E.
H.
R.
) system.
Position may routinely perform, with guidance additional duties consistent with the Health Information Representative I.
SPECIFIC DUTIES:
Scans and imports directed clinical documentation into E.
H.
R in a timely manner and with a high degree of accuracy.
Pulls Inventory paper charts as requested.
Files all returned charts in the appropriate place according to number.
Purge inactive paper files and do annual preparation of shredding of files beyond retention requirements.
Notifies Department Lead or Supervisor of any errors i.
e.
Information in either paper or E.
H.
R which does not match a patients indemnity.
Participates in meetings as required.
Assists other personnel as needed.
This is expected to also include certain duties generally associated with the Health Information Specialist II position, under monitoring and supervision.
Performs other duties as assigned.
MINIMUM REQUIREMENTS:
High school diploma/GED Previous typing/clerical experience is preferred Ability to communicate in Spanish and written and verbal is preferred but not required.
Ability to type 35 wpm.
Applicant to provide typing certificate.
Valid California driver's license or identification card, proof of insurance and personal transportation if driving.
PERFORMANCE REQUIREMENTS:
Knowledge, Skills and Abilities Ability to work well and concentrate in a busy office setting.
Ability to file (both alphabetically and numerically) with an outstanding degree of accuracy.
Ability to follow directions well without constant supervision.
Ability to establish and maintain effective working relationships with patients, employees And the public.
Ability to pay close attention to detail.
PHYSICAL REQUIREMENTS:
Must be able to lift up to 40 pounds and push up to 100 pounds (on wheels).
Must be able to hear on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
Must have vision with or without lenses that are adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
Must have high manual dexterity.
Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouch, reach, kneel, twist/turn, do fingering and feeling.

Estimated Salary: $20 to $28 per hour based on qualifications.
