Summary:
The Patient Care Coordinator position will manage the Intake and Scheduling of all patients.
This includes the reception and intake of new referrals, as well as maintaining all clinical field staff schedules for admissions, discharges, resumption of cares, recertification visits, and routine follow up visits as needed.
This position will also assist in offices activities such as processing DME, Demographic Change requests, verification of Therapy Vendor visits, Medical Supplies and lab results, and answering the incoming phone calls.
Responsibilities Manages the processing of referrals for all patients.
Manages all aspects of the clinical field staff schedules for all patients, while ensuring admitted patients are seen as ordered by the referring physicians and within 48 hours.
Assists with various department functions, duties, and activities in the PCC department as needed.
Ensures that all relevant patient information is obtained in an accurate and timely manner, and is entered into HomeCare HomeBase system, in order to meet MEDICARE requirements, when patients are referred to LORIAN by referral sources.
Regularly collaborate with the sales and clinical management team in reviewing new referrals and determining the appropriateness of any given referral for the agency.
Enter medications for new patient referrals.
Provides all relevant patient information (i.
e.
patient personal demographic, history and physical, current medications, physician's orders, F2F, surgical reports, etc.
) obtained upon referral to the Medical Records department for scanning into patient charts.
Assists as needed in processing DME, Medical supply requests.
Manages all aspects of Lab requests, including referrals to vendors and follow up to confirm the visit took place as well as faxing results to the appropriate Physician.
Ensures that all information is confirmed regarding current patients that are in the hospital.
Provides support to verify visits provided by Therapy Vendors in our EMR system.
Follow-up on hospital holds until the ROC visit is completed.
Monitors the coordination note workflows to manage Demographic Change Requests & DME requests.
Assists with processing scheduling requests from clinicians and patients that call Lorian.
Helps answer incoming phone calls.
May perform other duties as assigned.
Required Education and
Experience:
High School Diploma or Equivalent.
One (1) to two (2) years of experience in health care, Home Health preferred.
Additional
Qualifications:
Working knowledge of Medical terminology.
Strong knowledge of Microsoft Office Systems (i.
e.
Word, Excel, etc.
) and with an EMR system, HCHB is preferred.
Must have professional and customer-service-driven phone and communication skills.
Ability to communicate effectively and tactfully with management, clinical staff and office staff in a constructive, goal directed, and professional manner.
Ability to communicate effectively and tactfully with patients, doctors, and all customers of Lorian Health (LORIAN) in a productive, constructive, and professional manner.
Strong ability to organize and prioritize workload on a regular basis based on the quantity of incoming referrals.
Ability to be flexible and to follow verbal and written instruction in a fast-paced team-oriented environment.
Proficient in completing routine paperwork, multi-tasking, and providing appropriate follow-up as needed.
Outstanding interpersonal relationship building.
Maintains confidential patient information in accordance with privacy and security standards of the Health Insurance Portability and Accountability Act (HIPAA) and other applicable state laws.
Physical Requirements Ability to sit at a desk for long periods of time.
Ability to use a phone either by handset or by headset for long periods of time.
Ability to deal effectively with high levels of stress.
